Heartwood Treatment Centre Referral

If you would like a client or patient to receive treatment at the Heartwood Centre for Women, fill out the referral package and follow the steps listed on this web page.

Who is this program for?

The Heartwood Centre for Women is for women from across British Columbia, including members of Two-Spirit and gender-diverse communities, over 19 who live with severe substance use challenges and mild to moderate mental health concerns. The program is designed for those who have experienced several relapses, and whose treatment needs are beyond what local community health resources or health authority can meet.

Treatment at the Heartwood Centre for Women is voluntary. Women may be admitted to the program on leave from another care facility.

Referrals process

Step 1

Download the referral package.

Step 2

Follow the instructions and complete all the forms in the package.

This includes any appropriate supplementary forms. 

Step 3

Submit the package to your local health authority liaison.
