To request information from your medical file,  print and complete the Release of Information Authorization Form. Once complete, mail it to the relevant to the hospital or facility. You can find all mailing addresses in this list of contact information and addresses. 

If you are requesting records from Correctional Health Services, please connect with the health care team at the relevant correctional site to request a form.

Please note that due to the volume of requests received, it can take four weeks or more to process release of information. For privacy reasons, do not send personal medical information by email.
