Provincial Substance Use Program for Adults Referral

If you would like a client or patient to receive treatment at the Phoenix Society, fill out the referral package and follow the steps listed on this page.

Who is this program for?

Treatment at the Phoenix Society is for people who are 19 or older and have a substance addiction. Services are designed for people who have experienced several relapses, and whose treatment needs for substance use and addiction are more than what their local community health resources or health authority can meet.

While mental health support will be provided, the provincial beds at the Phoenix Society are not designed to treat mental health diagnoses. Treatment is voluntary.

Referrals process

Step 1

Download the referral package.

Step 2

Follow the instructions and complete all the forms in the package.

Step 3

Submit the package to your local health authority liaison.
