Forensic Psychiatric Services Pre-Sentence Report Referral

A pre-sentence report is used to help the court decide what sentence to give to someone who may have a mental health disorder. If this report has been requested, fill out the referral form and follow the steps listed on this page.

A person involved in a legal matter may be referred by the court for a psychiatric or psychological assessment. This assessment, combined with other information, allows the court to make an informed decision about the treatment and rehabilitation options an individual may need.

Step 1

Download the referral form.

Step 2

Complete and sign the form.

Step 3

Gather required supporting documents. 
The required documents are listed on the referral form.

Step 4

Mail or fax completed package to the desired location, as indicated on the form.
